Carbamazepine
Indications/Uses
Tonic clonic seizures; partial epilepsy. Resistant maniac depressive psychoses. Hypomania & mania. Trigeminal neuralgia & chronic pain.
Dosage/Direction for Use
Adult Initially 100-200 mg once daily or bid, may give small increments of 200 wkly, usually 400 mg bid or 600 mg bid until best response is obtained. Childn 10-15 yr Max: 600-1,000 mg once daily, 5-10 yr Max: 400-600 mg once daily, >5 yr 10-20 mg/kg once daily in 2 divided doses. Trigeminal neuralgia 600-800 mg for complete pain relief.
Administration
Should be taken with food: Take during or after meal. Swallow whole w/ little liqd, do not chew/crush.
Contraindications
Hypersensitivity. Previous history of acute intermittent porphyria. AV conduction blockade. Absence attack (petit mal).
Special Precautions
Not to be discontinued abruptly. Patients w/ serious heart disease, liver or kidney disorders. Perform LFTs & complete hemogram before starting therapy. Alteration in liver condition; occurrence of neutropenia, leucopenia, thrombocytopenia occur. May impair ability to drive & use machines. Pregnancy & lactation. Not recommended in childn <5 yr. Elderly.
Adverse Reactions
Sedation, ataxia, visual problems, headache & confusion; dry mouth, nausea, constipation, loss of appetite; skin allergy, serious exfoliative dermatitis, acne, blood dyscrasias, pedal edema.
Drug Interactions
OCs, anticoagulants, other antiepileptics, hormones, MAOI therapy, erythromycin, INH, Ca-channel blockers, cimetidine. Exacerbated CNS side effects w/ alcohol & psychoactive drugs. Increased lithium neurotoxicity.
